is the Delta Diabetes Center? |
The
Delta Diabetes Center (DDC) was formally established in September 1999 as a resource
that specializes in diabetes management and education. The mission of the Center
is to assist in the ongoing management and care of diabetes and prevent or halt
the debilitating complications associated with diabetes. The mission is achieved
by providing diabetes education, community programs, and outreach.
The Delta Diabetes Center of Missouri Delta Medical Center meets the National Standards for Diabetes Self-Management Education as defined by the American Diabetes Association and has been certified by the American Diabetes Association since 2002. This certification exemplifies that The Delta Diabetes Center is a provider of quality services and also allows reimbursement from Medicare and the majority of private insurances.

The
DDC is located at Missouri Delta Medical Center on the lower level. Appointments
are needed for outpatient education, but walk-ins are permitted
for general information.

Diabetes educators help you successfully manage your disease
It is easy to become overwhelmed and frustrated when you are dealing with the complexities of diabetes. If you find it difficult to make necessary lifestyle adjustments when you leave the doctor's office and return to your everyday routine, you are not alone.
Diabetes requires daily self-management - making healthy food choices, staying physically active, monitoring your blood sugar, and taking medications as prescribed by your doctor. Often, it is hard to stay on track and feel motivated.
Diabetes educators can help
Diabetes educators are health care professionals - nurses, dietitians, pharmacists, doctors, exercise physiologists, among others - who specialize in the care and education of people with diabetes.
As a key member of your diabetes management team, a diabetes educator will guide you through your treatment and counsel you on how to incorporate healthy choices into your life. Diabetes educators work with you to develop a self-management plan that meets your needs - one based on your age, school or work schedule, as well as your daily activities, family demands, eating habits, and health problems.
Diabetes educators can help you:
- Learn how to balance your eating, physical activity, medication, and blood sugar monitoring routines
- Develop healthy coping strategies and problem-solving skills
- Prevent or delay other complications such as heart disease, blindness, kidney failure, nerve damage, and physical intimacy problems
- Improve your confidence in your ability to manage your diabetes.

Obtain a referral for a diabetes educator
Diabetes educators give you the strategies and support you need to successfully live with your diabetes. Ask your doctor to refer you to a local diabetes educator for diabetes self-management training. It is covered by Medicare, many state Medicaid programs, and most health insurance plans.
