Notice of Data Incident

MDMC has discovered that an unauthorized third party had accessed its computer network. Upon identifying the issue, MDMC promptly initiated an internal investigation, has been in contact with law enforcement and retained a leading computer forensics firm to investigate the incident and to confirm the security of its computer systems. MDMC is not aware of any fraud or misuse of any individual’s personal information as a result of this incident.

MDMC recently determined that the unauthorized third party acquired certain files on August 22, 2021 that contained certain limited patient information. MDMC reviewed those files to identify what and whose information was contained in the relevant files.

On October 18, 2021, MDMC determined that the relevant files contained patients’ names, phone numbers, and certain medical information, such as the reason why the patient was seen at MDMC, the MDMC facility where the patient was treated and/or physician’s name. This incident did not involve patients’ Social Security numbers.

After determining whose information was contained in the files, MCMC searched for last known mailing addresses to promptly notify those individuals. MDMC sent written notification to the individuals whose personal information was contained in the files and for whom it has contact information. Individuals should refer to the notice they will receive in the mail regarding steps they can take to protect themselves.
